Abstract
110 accident cases were collected, based on the Human factor analysis and classification system (HFACS), the work trying to learn the active failure and latent failure in the Britain railway accidents. The generations of the human factor analysis method are reviewed, and the HFACS method was also illustrated the first analysis task of organizational influences, such as, the organizational process, climate and resource management factors. During 2020-2024, there were 15 freight and passenger train derailment, 23 occupation accident, 19 collisions, 15 SPADs and 38 accidents caused by other factors. In the accident analysis, the perception and decision error were frequently identified in the unsafe acts, the inadequate supervision and planned inappropriate operation learned in the unsafe supervision were contributed to the accidents. In the safety climate, the safety culture, equipment and facility resource in the resource management and the operation in the organizational process were the main reasons depicted the organizational influence. The revised Human Error Assessment and Reduction Technique (HEART) and utilization of Error Producing Conditions (EPCs) as the analysis method to be the comparison with the HFACS, to learn the overall failure probability of human factors in statistics analysis, the section was discussed in the fourth part.
